We are committed to ensuring the safety of our clients and their families. At HDST we have an incident management system in place to ensure that all incidents are appropriately identified and managed, and improvements are made where possible. As NDIS providers, HDST is required to inform the NDIS commission of any reportable incidents. These include occurrences and allegations of:

  • A serious injury or illness,
  • A dangerous incident that exposes self and/or others to serious risk
  • Abuse or neglect
  • Unlawful sexual or physical contact or assault
  • Sexual misconduct
  • Unauthorised use of a restrictive practice.

Steps undertaken by staff when managing incidents at HDST:

A reportable incident that has occurred with an NDIS participant needs to be reported to the NDIS Commission within 24 hours of management receiving information regarding the incident.
