We believe in transparency, so we will discuss and create a service agreement with all our clients. This ensures our clients have understood all important policies and procedures, and have agreed to a schedule of intervention and therapy goals.

Service Agreement Steps

Our service agreement with all new clients is created through two primary documents:

  1. Service Agreement Client Form – in Cliniko
  2. Support Plan Letter – in Cliniko

The process of generating these documents is detailed in the flow chart. The intake team, admin team, and all therapists play an integral part in ensuring a service agreement is made in a timely manner. Our admin team are responsible for ensuring that both NDIS and private paying clients are aware of all critical policy and procedure, including provision of non face-to-face services, and our fee schedule for the year.

Our therapists will be able to generate their client’s support plan using the ‘letters’ function in Cliniko. This allows for therapists to edit the letter as needed, including omitting the goals section for private paying clients. A signature is not required for this letter but therapists are required to email this directly to clients through Cliniko and ensure their client has accepted the support plan.

All new clients will be sent this form by our admin team upon making an initial booking. There are different forms sent to clients depending on whether they are NDIS funded or private paying.

Therapists are responsible for completing this letter for clients engaging in ongoing services with HDST. This letter can be updated when needed, emailed to clients and also printed for attaching to emails.
